2Deaths were considered "Diabetes, any-mention", if either the underlying or related-cause of death were coded E10-E14.
3Chronic kidney disease with ICD-10 codes N03–N05, N13.0–N13.3, N17–N19, N25–N26, N28.0, and N28.8.

arthritis and other rheumatic conditions
.
6Discharges with a diagnosis of Diabetes mellitus (ICD-9CM code 250) without mention of traumatic injury (895-897), and with procedures for major or minor lower extremity amputation (84.1).

Causes of death are classified in accordance with the Tenth Revision of the International Classifications of Diseases (ICD-10) ; cancer incidence with Third Revision of the International Classifications of Diseases (ICD-O-3) ; and hospital discharges with International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM). Codes are grouped into broader categories for the causes listed in these tables in order to classify the causes of death or incidence of disease. The Council of State and Territorial Epidemiologists under the sponsorship of the National Center for Chronic Disease Prevention and Health Promotion defined code groupings in order to classify diseases with substantial public health burden and consistent with Healthy People 2010 goals. See the Chronic Disease Indicators Home for code-grouping definitions and further indicator statistics.
