Application Forms to Correct or Change Michigan
Birth and Death Records

If you wish to make a correction or change to a Michigan birth or death record filed with the State of Michigan Division for Vital Records, you will need to submit an official application. The application must include the correct payment and in most situations appropriate supporting documentation. Please review the following list of applications to change Michigan birth record and death records. Each application includes instructions on how to complete the form, information on who is eligible to request the correction or change, the documentation that is required, and the appropriate fee payment. The following three application forms are to be used to make specific changes to a Michigan birth record, such as adding a father or making a legal name change. Applications for making more general corrections or changes to errors and omissions on a Michigan birth record and applications to make corrections to a Michigan death record are listed further down. Please review the entire list below to determine the appropriate form for your needs.
  • Add a Father
  •   This form is to be used to add a father when there is no father currently listed on the birth record.
  • Remove or Replace a Father
  •  This form is to be used to remove a father from a birth record. This form may also be used to replace the previously named father.
  • Legitimation
  •  This form is to be used if a change is required to the name of a child as a result of the parents' marriage.
  • Court-Ordered Legal Name Change
  •  This form is to be used for making name changes that are directed by court order.


    The following three forms are to be used to request general corrections needed because of errors or omissions on a Michigan birth record.
  • Child Less than One Year Old
  •  This form is to be used to correct errors and omissions on a Michigan birth record of a child who is currently younger than one year of age.
  • Child Now Age One through Five
  •   This form is to be used to correct errors and omissions on a Michigan birth record of a child who is currently ages one through five.
  • Person Now Age Six or Older
  •   This form is to be used to correct errors and omissions on a Michigan birth record of a person who is currently age six or older.

     

    The following forms are to be used only by public agencies or hospitals to make changes or corrections to a Michigan birth record.


  • Application to Change a Birth Record - Public Agency Use
  •  This form is to be used by a Michigan public agency to make corrections and changes to a Michigan birth record. The agency must be the legal guardian of the child.
  • Application to Change a Birth Record - Hospital Use Only
  •  This form is to be used by a Michigan hospital to make corrections and changes to a Michigan birth record.

  • Delayed Registration of Birth - Hospital Use Only
  •  This form is to be used by a Michigan hospital to register a birth that was not registered within the first year.

     

    The following forms are to be used to correct a Michigan death record.


  • Application to Correct a Michigan Certificate of Death
  •  This form is to be used by the next-of -kin or funeral licensee only.

  • Physician Application to Correct a Michigan Certificate of Death
  •  This form is to be used by the physician who certified the death.
